One caveat first: a valid EICR must be issued on the BS 7671 model forms by a competent person, with every circuit's results recorded and each observation coded, so a blank Word template is a study aid rather than a certificate you can hand a landlord. With that said, here is the full structure.
What an EICR must contain
- Client and installation address, and the purpose of the report
- Details of the installation: occupier, estimated age, evidence of alterations, date of last inspection
- Extent and limitations of the inspection, agreed with the client in writing
- Summary of the condition of the installation, and the overall assessment: Satisfactory or Unsatisfactory
- Observations, each given a classification code: C1, C2, C3 or FI
- Recommended date for the next inspection
- Schedule of inspections: the checklist of items inspected
- Schedule of circuit details and test results: Zs, R1+R2, insulation resistance, RCD operating times and more
- Declaration with the inspector's name, signature, qualifications and scheme membership
